  • Admission for service, given our agency’s ability to provide suitable care;
  • A healthcare provider who is both knowledgeable and skilled in the specific care program you require;
  • Involvement in the design, planning, and administration of care programs, routine reviews, care adjustments, and personnel changes that our agency may implement;
  • Awareness of the schedules of your care program and the identities of the individuals who will be part of it;
  • The option to request a specific care provider and decide the duration of their services;
  • The right to refuse any or all treatment, as far as the law permits;
  • Referral to other services if denied services for any reason or upon request;
  • Assistance in seeking community resources that can offer financial support for your program;
  • Information about the procedures for submitting written complaints or grievances;
  • Appropriate responses to any complaint or grievance submitted; and
  • Access our contact information and service schedules to acquire additional information about our agency, lodge complaints, or report abuse.
